Thuốc có nguy cơ gây tăng huyết áp

Cơ chế gây tăng huyết áp có thể liên quan đến tăng trương lực của mạch, do hiện tượng thiếu oxy giảm khi dãn mạch và sự giải phóng NO [1]. Tác dụng gây THA do erythropoietin phụ thuộc liều [5]

Tacrolimus có ghi nhận gây THA nhưng tỉ lệ gặp phải thấp hơn Cyclosporin (35% so với 50% đối với Cyclosporin) [4]. Do đó, có thể xem xét chuyển sang sử dụng Tacrolimus đối với bệnh nhân THA do cyclosporine (Ehud Grossman, 2004) [5,4]

Gây co mạch trực tiếp do giảm hình thành nitric oxid

Bệnh nhân thường bất ngờ khi họ biết rằng thuốc họ đang dùng có thể gây tăng huyết áp. Cơ chế của tăng huyết áp có thể do tăng giữ thể tích, kích thích dẫn truyền cường giao cảm hoặc co mạch [10,8]. Mặc dù chỉ có một vài nhóm thuốc có thể gây tăng huyết áp đáng kể trên lâm sàng, các nhân viên y tế (đặc biệt là bác sĩ và dược sĩ) cần nắm thông tin về chúng để kiểm soát và can thiệp khi tình trạng tăng huyết áp do thuốc xảy ra. Dù rằng phần lớn trường hợp các thuốc sẽ không gây tăng huyết áp trầm trọng nhưng ở một vài bệnh nhân có thể quan sát thấy huyết áp tâm thu tăng 5-10 mmHg [8].
